Care transition from the theoretical perspectives of Afaf Meleis, Mary Naylor, Eric Coleman, and Eric Cassell

Objective: To understand the relationship between the theories of Afaf Meleis, Eric Coleman, Mary Naylor, and Eric Cassell in the transitional care process within the hospital context. Methods: This is a narrative review with a qualitative approach, guided by SANRA. The search was conducted in LILACS, MEDLINE, PubMed, and SciELO, as well as Scopus Preview and Science Direct, in January 2026, including studies published in the last five years in Portuguese, English, and Spanish. The selection followed PRISMA recommendations. Results: The predominance of Afaf Meleis’ Transitions Theory was observed as the most frequently identified theoretical framework, highlighting the central role of nursing in coordinating transitional care and in understanding the adaptive processes experienced by patients and their families. Complementarily, the models of Eric Coleman and Mary Naylor emphasize strategies focused on patient empowerment, care self-management, and interprofessional coordination during hospital discharge and post-discharge follow-up. Furthermore, Eric Cassell's philosophical perspective expands this understanding by highlighting the centrality of the experience of suffering and the person's uniqueness in the care process. Conclusion: The integration of these theories demonstrates that healthcare becomes more comprehensive and effective when different perspectives are articulated.
