Sensitivity and specificity between the Alvarado’s and Ripasa scales for the diagnosis of acute appendicitis emergency service

Abstract

Objective: To determine the level of sensitivity and specificity between the Alvarado and Ripasa scales in patients who present with symptoms suggestive of acute appendicitis to the Emergency Service of the Hospital Central Universitario ¨Antonio Maria Pineda¨. Materials and methods: The present investigation corresponds to a quantitative approach, expofacto, field, non-experimental, retrospective, cross-sectional. The population consisted of 85 cases with a postoperative diagnosis of acute appendicitis and the 85 medical records were reviewed. Results: In this study, the female sex prevailed over the male. The most frequent consultation symptom was pain in the right iliac fossa, the age was between 21-30 years. The location of the appendix that prevailed was internal Laterocecal. According to the type of appendicitis, Phlegmonous Appendicitis was more frequent. Among the complications, leukocytosis was predominant in most of the patients. The Alvarado scale showed a sensitivity of 87% and specificity of 89%, while the RIPASA score reported a sensitivity of 97% and a specificity of 93%, the Alvarado score with a higher positive predictive value than the RIPASA score. RIPASA, respectively 94% over 92%. Conclusions: The RIPASA score behaved with greater sensitivity and specificity than that of Alvarado.
